Determining the appropriateness and safety of utilizing neuromuscular stimulation (NMES) as a complementary therapy for the recovery of patients undergoing vascular and general surgical procedures.
A single-center, prospective, randomized, single-blind, parallel-group controlled study. This single-centre study will be conducted in the UK, at a secondary care National Healthcare Service Hospital. Vascular and general surgical patients, 18 years or older, with a Rockwood Frailty Score of 3 or greater when they enter the hospital. An unwillingness or inability to participate in the trial, coupled with implanted electrical devices, pregnancy, and acute deep vein thrombosis, all represent exclusions. The recruitment goal is set at a hundred. In preparation for surgery, participants will be randomly assigned to either the active NMES group, labeled Group A, or the placebo NMES group, designated as Group B. Daily, blinded participants will employ the NMES device (30 minutes per session), from one to six times, post-surgery, supplementing standard NHS rehabilitation, until discharge from care. Patient satisfaction with the NMES device, as determined by discharge questionnaires, and any adverse events reported during hospitalization, indicate the acceptability and safety of the treatment. Postoperative recovery and cost-effectiveness are secondary outcomes evaluated in both groups through varied activity tests, assessments of mobility and independence, and questionnaire results.

The EDDIE+ program, a theory-driven, multifaceted intervention, equips nursing and personal care staff with the skills and authority to recognize and address early signs of decline in aged care facility residents. A stepped wedge randomized controlled trial will be accompanied by an embedded process evaluation aimed at determining the fidelity, acceptability, mechanisms of action, and contextual barriers and enablers inherent in the EDDIE+ intervention.
Participating in the study are twelve RAC homes situated in Queensland, Australia. A thorough mixed-methods evaluation, guided by the i-PARIHS framework, will be conducted to evaluate intervention fidelity, contextual influences, the mechanisms of action, and the acceptability of the program according to various stakeholders' perspectives. Utilizing project documentation, quantitative data will be gathered prospectively, encompassing baseline site context mapping, detailed activity logs, and regular communication check-in forms. Qualitative data collection, post-intervention, will employ semi-structured interviews with varied stakeholder groups. A structured analysis of quantitative and qualitative data will be performed, employing the i-PARIHS constructs for innovation, recipients, context, and facilitation.

Iron and folic acid (IFA) supplements, proven to ameliorate anemia in pregnant women, unfortunately experience a low utilization rate in Nepal. Our supposition was that the twice-monthly provision of virtual counselling during mid-pregnancy, compared to antenatal care alone, would result in improved compliance with IFA tablets during the COVID-19 pandemic.
This non-blinded, individually randomized controlled trial in the Nepalese plains assesses two intervention groups: (1) standard antenatal care; and (2) standard antenatal care combined with virtual counseling sessions. For enrollment purposes, pregnant women who are married, within the age range of 13 to 49, who are capable of responding to questions, whose pregnancy is between 12 and 28 weeks, and who plan to live in Nepal for the next 5 weeks are eligible. Two virtual counseling sessions, separated by at least two weeks, are part of the intervention, and are led by auxiliary nurse-midwives, focused on mid-pregnancy. Pregnant women and their families benefit from the dialogical problem-solving method employed in virtual counselling. Returning home after an emergency department (ED) visit poses significant challenges for older adults with frailty, due to multiple interacting physical and social problems. Biomimetic scaffold In-home assessments and interventions, incorporated into paramedic supportive discharge services, help navigate these challenges. Existing paramedic programs intended to assist with patient discharge from the ED or hospital, thus averting unnecessary hospitalizations, are the subject of this description. An extensive analysis of existing literature on paramedic supportive discharge services will provide (1) a justification for these programs, (2) details on the recipients, referral points, and service delivery teams, and (3) specifics on the assessment and intervention strategies employed.
We intend to integrate studies that examine enhanced paramedic capabilities (community paramedicine) and the expanded scope of care for individuals transitioning from emergency departments or hospitals after discharge. All study designs, spanning all languages, will be considered for inclusion. A targeted search of grey literature, combined with peer-reviewed articles and preprints, will be included in our research, spanning the period from January 2000 to June 2022. The proposed scoping review will follow the procedures detailed by the Joanna Briggs Institute methodology.
